What This Site Is

This site is an informational property of Cadence, LLC dba Continuum. It describes Continuum's virtual SUD treatment-continuity model, its evidence base, and its pilot program for self-funded employers, health plans, and brokers. The site itself does not deliver clinical care, and using it does not create a patient or treatment relationship of any kind.

Not Medical Advice

Projected Figures

Performance figures identified on this site as targets, such as Continuum's retention targets, are model projections, not observed results. National baseline figures are cited to their published sources in the references section of the relevant page.

Chat Assistant

The site's chat assistant is automated. Its answers are generated by software, may contain errors, and are limited to describing Continuum's model. They are not medical, legal, or financial advice, and no clinician reviews them.

Intellectual Property

The content of this site, including the Continuum name, the navigator-continuity episode model, and the materials describing it, is the property of Cadence, LLC. Cited third-party research belongs to its respective publishers.

The site links to outside resources, including published research and a third-party scheduling service. Those sites operate under their own terms and policies, and we are not responsible for their content.

Governing Law

These terms are governed by the laws of the Commonwealth of Kentucky. Any disputes arising from the use of this site shall be resolved in the courts of Jefferson County, Kentucky.
