This site, continuumhealth.care, is a marketing and informational property of Cadence, LLC dba Continuum. It describes Continuum's SUD treatment-continuity model for employers, health plans, and brokers. This policy covers what the website collects and how it is used. It is deliberately short, because the site collects very little.
Contact form. If you submit the contact form, we receive the fields you enter: name, email, and optionally company, role, and a message. Submissions are transmitted over HTTPS and stored in a secured, access-controlled database. We use them to respond to your inquiry and for no other purpose.
Chat assistant. The site includes an automated chat assistant that answers questions about Continuum's model. Messages you type are sent to our chat service, which uses a third-party AI provider to generate replies. The conversation lives in your browser for the current visit and is not tied to your identity. Please do not enter personal health information in the chat.
Scheduling. The "schedule a call" button links to Cal.com, a third-party scheduling service. Information you enter when booking is handled under Cal.com's own privacy policy.
Server logs and fonts. Like any website, our hosting provider processes standard server logs (IP address, browser type, pages requested) to deliver the site and protect against abuse. Fonts are served by Google Fonts, so your browser requests font files from Google's servers when a page loads.
Display preference. A few demonstration pages store a theme preference in your browser's local storage. It never leaves your device, and you can clear it through your browser at any time.
This website is informational. It is not a treatment service, and no SUD treatment records are created, received, or stored through it. When Continuum delivers clinical services, patient records and protected health information are governed by separate treatment, consent, and payer agreements, including, where applicable, HIPAA and 42 CFR Part 2, the federal confidentiality rules for substance use disorder treatment records. Nothing you do on this website creates a patient relationship or a Part 2 record.
Contact form submissions are retained while we are corresponding with you and for a reasonable period afterward for business records. You can ask us to delete your submission at any time.

If the site begins collecting anything new, this policy will be updated before that happens.
